Prepping for your initial appointment

• Please arrive 15 minutes early for your first appointment.
• Bring a copy of your insurance cards and a photo ID with you.
• Please call our office at (954) 369-5787 to verify your insurance benefits beforehand.
• Have in writing key past medical history, previous treatments, current medications, past surgeries, and other important information.
• List out signs and symptoms that can be of importance to the therapist. What has been hurting or bothering you? For how long?
• It’s a good idea to make a list of questions that you have to discuss on your first visit.

Another benefit of going directly to our physical therapists is the cost savings. Beyond the cost of copayments to see your physician or being sent to a specialist paying another copay, and finally getting sent to physical therapy, you can save money and see our team directly.

At, Dr. Physio Therapy & Wellness we are in-network with Medicare. We are able to accept commercial insurances when is a PPO policy with out-of-network benefits. A copay per visit might be implied.

For patients who do not have health insurance or have exhausted their benefits, our Fort Lauderdale, FL physical therapy clinic also offers a private pay plan – we charge a self-pay fee of $90.00 per visit.
